Supported Vacations
$620/week
includes all transportation, all meals and entertainment and staff support.
Applicant Requirements:
Individuals must currently be supported by Community Living Hamilton.
Individuals must be 15 years of age or older
Individuals must be flexible to vacation any time between May 15 and September 30th.
Application Process
Application Packages are mailed out January 11th, 2008
You must return completed applications between January 14th to February 29th, 2008
Applicants will be notified of their acceptance by April 15th, 2008.
Acceptance
Your acceptance to the program and on a particular week is based on many factors. We do our very best to make certain everyone has the best vacation possible. We take into consideration all of your interests, abilities and wishes for your week away; then we endeavour to match you with other applicants who share these factors with you. It is very important that we learn as much about you as possible from your application form, so please take your time in completing the application and send additional notes if needed.
Payment
$50 deposit is due with your completed application form by February 29th, 2008.
Final payment is due May 16th, 2008.
