Preschool Services:  Child Development Centre

As a participating member of the Integration Resources HUB, Community Living Hamilton provides Special Needs Resourcing supports to facilitate community child care settings.  As well, staff assist families with information, referrals and the eventual transition to Board of Education programs.  More information related to child care services is available through “Coordinated Access for Child Care at www.cafee.on.ca

Special Services at Home Coordination

SSAH  enables families to access funding from the Ministry of Community and Social services to have a worker support their son\daughter in their home or community for a few hours a week.  Community Living Hamilton offers assistance to families in applying to the program  as well as in the administration of approved contracts (eg: recruiting, hiring, supervision and payment of a worker)

Charlton Family Support Program

i)  Out - of - Home Respite Care:
Charlton House provides occasional out-of-home- respite care for individuals under the age of 21 years.  The program provides a range of recreational\leisure activities designed to provide the child/adolescent with an enjoyable social experience while at the same time meeting their family’s need for respite.  Charlton House is open every weekend, as well as full time during the Christmas, March and Summer school breaks.

ii)  Adolescent Groups:
Individuals between the ages of 15 and 21 years of age are eligible to participate in small group sessions offered from October to January and February to May each year.  Participants attend one evening a week and are involved in a range of activities focused on socialization and life skills development.  Groups also include special events (eg:  dinner dances, trips, etc.)  as well as focus groups on topics of interest to young adults (eg:  relationships, sexuality).

Alternate Care Program

The Alternate Care Program provides a family based “shared care” model of support for individuals under the age of 18 years and their families.  Families/individuals are recruited from the community to provide care to a child/adolescent in their home for up to a maximum of 10 days per month.
